Service Costs

• We utilize insurance for fees associated with services whenever possible. Co-pays, co-insurance and deductible payments are due at the time of service. 

• Or, you may elect to pay service costs yourself.  If you do so, you have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Click here to read more: GFE 

• Payments are collected at the time of each service via credit or debit card through a privacy compliant payment system. Alternatively, you can pay by cash or check. 

• We are IN-NETWORK with: Medica, United Healthcare UBH/UHC, Medical Assistance (MA/PMAP/Medicaid), Medicare, Blue Cross Blue Shield MN/ND, Ucare, PrimeWest, Health Partners, Preferred One, AllOne Health EAP, Health Advocate EAP & Sanford Health Plan

• Please contact us to find out if you have questions about whether we are in-network with your insurance

• If we are not in-network with your insurance, a reduced fee option may be available to you based on income. Please ask if you think you may qualify. Some insurances provide out-of-network benefits, call your insurance provider to find out if you are eligible; a superbill can be provided if needed.
